Case Management

Brief Description

case-management3Case management services provide for the coordination of services to adults and their families/guardians. Services include meeting people to discover the person's/family's needs, developing individualized service plans to address needs, advocacy to develop services identified in the plan, and progress monitoring and evaluation.

Service Location

Case management services are provided through office visits, home visits, school visits, and a variety of other community locations where the adults need assistance and coordination.

Who Qualifies

Adults who meet the definition of intellectual disability, pervasive developmental disorder, or an AXIS I mental health diagnosis, AXIS II (Personality Disorders); or those who have been determined to be at risk of mental impairment due to established environmental or biological risks as determined by the Dept. of Health and Human Services.

Who Refers

Department of Health and Human Services, school system personnel, direct service/home health agencies, direct referrals from families.

The Department of Health and Human Services (Medicaid)

Service Outcome

Adults receive coordinated supports to meet their unique needs; families are supported in ways that allow their children to be successful in their homes, schools, and communities; communities increase their capacity to care for its families and children in need.
