Brief Description
Shared Living is a Home Support option for people receiving services under Section 20, 21 or 29. The individual lives in a home with a full-time support provider. For KFI, Shared Living usually happens with a family member or close connection serving as the provider. The Shared Living provider is a trained DSP and a contractor for Home Support services with KFI as the administrative oversight agency. The individual receiving Shared Living services may receive assistance with household tasks, medication management, independent living skills, accessing community resources, making connections in the community and building and strengthening relationships. The supports are customized to meet the individual’s needs and desires.
KFI loves to work with families who are committed to helping the person they support live their best life with as much independence and autonomy as possible. We work with the team to hold up a shared vision of the future for that individual. KFI Shared Living Coordinators connect with the individual and provider each month, with in-person visits at least every other month. Shared Living Coordinators can serve as a resource to help with planning, making connections in the community and helping people to fill valued roles. KFI offers assistance with Futures Planning and Person-Centered Planning tools such as PATHs, MAPs, Liberty Plans, etc. to help people realize their goals and plan for the future.
Our mission is the same for people who choose Shared Living as it is for the people we provide direct services to. We support individuals to lead regular lives in their communities: The same, not different. We are committed to helping people have rich lives with lots of choices, autonomy and opportunity.
